Documentation requirements

Documentation provides Virginia Commonwealth University with verification of a student’s disability. Therefore, it is your responsibility as a student to provide documentation of your disability to the Office of Student Academic Support Services and Disability Support Services.

Documentation requirements vary depending upon the specific type of disability. In general, the less obvious the disability, the more information is required to determine a student’s needs and make accommodation recommendations. Documentation should:

  • Be recent.
  • Be relevant.
  • Be comprehensive.
  • Contain test scores and interpretations when appropriate (learning disability reports, audiograms, etc.).

If the original documentation is incomplete or inadequate to determine the extent of the disability or reasonable accommodations, the university may request additional documentation.
